If you prefer to take your exam at a testing center, you can find the closest Promoteric testing center from their website (prometric.com). Usually there's at least one Prometric testing center in most major cities in the US and around the world.
To register for your exam, start by visiting USGBC’s website (usgbc.org) and create an account if you don't have one. In your account, under the "Credentials" section, choose LEED Green Associate and follow the instructions on the screen to complete your registration. When registering for the exam, make sure that your name is written exactly as it appears on your ID that you will be presenting on the the exam.
